PHP Warning:  PHP Startup: Unable to load dynamic library '/usr/lib/php5/20090626+lfs/sqlite.so' - /usr/lib/php5/20090626+lfs/sqlite.so: cannot open shared object file: No such file or directory in Unknown on line 0
PHP Warning:  require_once(/var/www/urd/urdd/../config.php): failed to open stream: No such file or directory in /var/www/urd/urdd/urdd.php on line 35
PHP Fatal error:  require_once(): Failed opening required '/var/www/urd/urdd/../config.php' (include_path='.:/usr/share/php:/usr/share/pear') in /var/www/urd/urdd/urdd.php on line 35

the first line is typically a PHP configuration error. Presumably a update that left an older php.ini file that also loads the mysql library.
the second seems to be missing some package, that it wants to load. I'm not sure why that happens. Perhaps broken defendencies in packages. Assuming you run a  ubuntu/debian like system try sudo apt-get -f

The last error means that a file could not be found, please check if it exists, if it doesn't the urd installer probably didn't finish correctly. Possibly due to a permissions error.

Thank you for replaying spearhead.
Yes i'm running ubuntu server 11.10 and yes i've tried apt-get -f install, but nothing. I will retry with a fresh installation.
Thank you again.

Seems your not alone with the sqlite thing https://bugs.launchpad.net/ubuntu/+source/php5/+bug/875262. The workaround is in it too.
